Evaluation of We Choose Health, the Illinois Community Transformation Grant Initiative


This grant supported the evaluation of We Choose Health (WCH), a five-year statewide Community Transformation Grant awarded by the Centers for Disease Control and Prevention to the Illinois Department of Public Health (IDPH). The Community Transformation Grant program was abruptly ended before completion in 2014.

We Choose Health was designed to reduce the burden of chronic disease through the implementation of evidenced-based interrelated interventions that make "the healthy choice the easy choice." Communities throughout Illinois were responsible for the development of programs and interventions tailored to their needs. The IHRP-based evaluation team utilized population surveys, program data, and other measures to evaluate the implementation processes and outcomes of the Illinois community's efforts to improve community health status. In addition, targeted evaluations of select WCH strategies were to enhance the existing evidence base for strategies that promote tobacco-free living, active living and healthy eating, and quality clinical and other preventative services.

The Illinois program was one of 35 states and communities to receive funding under the CDC's CTG Initiative to encourage the transformation of communities towards improved health and wellness.
