Impact and Dissemination Evaluation of the Illinois Chronic Disease Self Management Program (CDSMP)

Abstract

In this project, we conducted a comprehensive impact evaluation of the Illinois Chronic Disease Self Management Program (CDSMP) initiatives. CDSMP is a widely distributed community based program that helps older adults to adopt healthy life styles and better manage their chronic illnesses. While CDSMP has demonstrated efficacy and effectiveness, to date, no evaluation has examined it with respect to adoption and maintenance in the community.

This evaluation is the first to evaluate the impact of CDSMP, including its reach, effectiveness, adoption implementation and maintenance across agencies that deliver the program. Findings from the evaluation provide critical evidence on how agencies and practitioners can more effectively implement and maintain CDSMP in health care settings. Findings also provide important information and tools for agency self-evaluation regarding the adoption of similar evidence-based health programs for older adults in community settings.
