All Inclusive Care Evaluation

Dr. Hughes will take primary responsibility for describing the implementation and operation of each of the three models of all-inclusive care. This includes documenting the structural and organizational characteristics of each model; examing the relationship of these characteristics to the processes of care, and monitoring the success of the implementation process for the duration of the evaluation. Dr. Hughes will site visit each of the three demonstration sites each year (during the 3 year demonstration period) to collecct this information. Between site visits, she will confer regularly with site participants on facilitators and barriers to implementation, and process of care issues. She will also work with the principal investigators on tracking health care utilization for demonstration and comparison subjects. Dr. Hughes will participate in team meetings, writing the report due to Congress, and in the development of manuscripts that ensue from this work.
